Introducción: La adherencia terapéutica se refiere al proceso a través del cual el paciente lleva a cabo las indicaciones del terapeuta, basado en las características de su enfermedad, de la relación que establece con el profesional de salud y de sus características psicológicas y sociales.
Objetivos: Identificar los factores de riesgo que influyen en la no adherencia al tratamiento farmacológico antihipertensivo en pacientes pertenecientes a los consultorios 17 y 18 del policlínico Asdrúbal López, de Guantánamo, en el año 2011.
Diseño Metodológico: Se realizó un estudio analítico de casos y controles. El universo del estudio estuvo constituido por los 164 pacientes hipertensos dispensarizados en el área y diagnosticados según el test de Morinsky-Green. Los casos constituyeron el grupo no adherentes al tratamiento antihipertensivo farmacológico (n=36). Los controles se seleccionaron de forma aleatoria simple, constituyendo el grupo adherentes al tratamiento antihipertensivo farmacológico (n=72), en una relación 1:2.
Resultados: Ventitrés pacientes de los casos (no adheridos al tratamiento) pertenecientes al sexo masculino, para el 63,9 % (p<0,05; OR = 2,62); 66,7 % de los casos no tienen conocimiento acerca de su enfermedad (p<0,05; OR=2,65); 77,8 % de los casos están bajo un tratamiento complejo (p<0,001; OR=3,31); 25 pacientes del grupo de los no adheridos al tratamiento manifestaron una relación médico-paciente no favorable, para el 69,4 %. (p<0,001; OR=4,84).
Conclusiones: El sexo masculino, el desconocimiento sobre la enfermedad y la complejidad del tratamiento, constituyen factores de riesgo en la aparición de la no adherencia al tratamiento farmacológico antihipertensivo.
ABSTRACT
Introduction: The therapeutic adherence refers to the process through which the patient carries out the therapist's indications, based on the characteristics of his or her illness, of the relationship that he or she establishes with the professional of health and of his or her psychological and social characteristics.
Objective: To identify the factors of risk that have an influence on the non-adherence to the pharmacological antihypertensive treatment in patients belonging to the clinics 17 and 18 of “Asdrúbal López” polyclinic center, of Guantánamo in the year 2011.
Methodological design: An analytic study of cases and controls were carried out. The sample was constituted by the 108 hypertensive patients dispensed in the area and who were diagnosed through Morinsky-Green's test. The cases constituted the non-adherent group to the antihypertensive pharmacological treatment (n=36). The controls were selected in an aleatoric simple way, constituting the adherent group to the antihypertensive pharmacological treatment (n=72), in a ratio of 1:2.
Results: 23 patients of the cases (not stuck to the treatment) belonging to the masculine sex for 63.9% (p < 0.05; OR = 262); 66.7% of the cases does not have knowledge about their illness (p < 0.05; OR=265); 77.8% of the cases is under a complex treatment (p < 0.001; OR=331) 25 patients of the group of those not stuck to the treatment did not have a favorable doctor-patient relationship of 69.4%. ( p < 0.001; OR=484).
Conclusions: The masculine sex, the ignorance about the illness, and the complexity of the treatment constitute factors of risk in the manifestation of the non-adherence to the pharmacological antihypertensive treatment.
